A dispatch service needs a written agreement with each carrier that makes clear you act as the carrier's agent, not a broker, plus a way to collect the carrier's authority, insurance and payment details before the first load. This pack gives you those documents as editable files written around FMCSA's 2023 guidance on dispatch services and brokers.

What's included
- 01_Dispatch_Service_Agreement (.docx + .pdf, 5 pages): Appointment as agent, services, no-brokerage terms, the carrier's right to refuse any load, fee and payment, insurance and liability, confidentiality, termination, signatures, Exhibit A (Load Scope) and Exhibit B (Fee schedule).
- 02_Limited_Authorization_to_Act (.docx + .pdf, 2 pages): Lets you talk to brokers and sign rate confirmations in the carrier's name only after the carrier approves each load. Has an expiry date and a revocation notice.
- 03_Carrier_Onboarding_Checklist (.docx + .pdf, 2 pages): Authority status, BOC-3, UCR, insurance, W-9, NOA or factoring letter, equipment, ELD, drivers and the broker setup packet.
- 04_Carrier_Information_Sheet (.docx + .pdf, 2 pages): Company, insurance, payment, equipment, driver and lane-preference details.

Compatibility: what we tested
This pack has no spreadsheets. On Oct 11, 2026 we opened all five .docx files in LibreOffice and converted them to PDF; page counts matched the included PDFs (5, 2, 2, 2, 2). Our product QA notes record a Google Docs import of files 01, 03 and 05 on Oct 7, 2026 with headers, footers, tables, checkboxes and signature blocks intact. We have not opened these copies in Microsoft Word for this page. These are templates, not legal advice: have a licensed attorney in your state review them before use.

Example in use
From the START HERE guide: open each .docx and replace every [bracketed] item, starting with [Dispatcher Company Name] in the header (Ctrl+H in Word, Edit > Find and replace in Google Docs). Fill in Exhibit A (Load Scope) and Exhibit B (Fee schedule) for each carrier. After your attorney approves the set, the carrier signs 01, 02 and 05, you collect the items on 03, and you complete 04 together.
